Psychiatric Rehabilitation and Recovery – Situation of Hong Kong

Abstract
Psychiatric rehabilitation and recovery are vital to the path of person suffering a mental illness, particularly persons with severe symptoms and complex needs. Contemporarily there are different models and practices across the world. Nevertheless, among the practices, the key components in recovery are empowerment, hope and healing. The recovery process is understood to be unique to the individual. In large part it is the discovery of a purpose and meaning of life. It calls for active involvement of the individual in daily life, the assumption of personal responsibility, the exercise of choice and a degree of taking risk. Importantly, there is no assumption of ‘cure’ in the sense of entirely escaping symptoms or impairments; instead, the emphasis is on achieving a fulfilling existence despite enduring disability. In Hong Kong, rehabilitation service has been enhanced in the past decade but there is much room for further improvement.

精神復康和復元-香港的情況

精神復康和復元對於精神病患者,尤其是症狀嚴重和需求複雜者的復康之路是非常重要的。現今世界各地有不同的模式和做法。然而,綜觀各種做法,復元的關鍵植根於授權,希望和治愈。復元過程對每個人來說都是獨特的。在很大程度上,復元是幫助患者發現生活的目的和意義。復元要求當事人在日常生活中積極參與,承擔個人的責任,作出選擇,並承擔一定程度的風險。更重要的是,這模式在未有假設病患者能被徹底“治愈”的前提下,強調當事人要活出一個充實的生活。在香港,復康服務雖然在過去的十年中已有相當提升,但仍有很大的進步空間以實踐復元的理念。

Promotion and Implementation of Recovery-oriented Practice in a Mental Hospital in Hong Kong

Abstract
Objectives: As an essential direction of mental health service reform, recovery-oriented practice has recently been widely incorporated into the guiding principles for the transformation of mental health systems in many countries.
Methods: A Committee on Recovery-oriented Practice was set up to steer the promotion and implementation of recovery policy in the Castle Peak Hospital, a major mental hospital in Hong Kong.
Results: Recovery-oriented Practice was promoted and staff training and patient education on the concept and principles of recovery were launched, and at least 300 staff members had participated in the training. Baseline data were collected on the attitude and acceptance of staff and patient on mental health recovery. Pilot recovery programs were adopted in the rehabilitation unit and gradually extended to other units.
Conclusion: The report demonstrated that Recovery-oriented Practice could be successfully implemented in mental health services for Chinese populations.

 

摘要

「康復進程」在一所精神科醫院的推行

目的:*「康復進程」乃全球精神科服務的重大方向。世界各地均以此為其精神科服務改革的政策。

方法:青山醫院成立了跨職系的委員會,為「康復進程」的推行制定了方向。

結果:委員會舉辦了多項為醫院的專業人員及病人而設有關「康復進程」的培訓及教育。至今共有多於三佰位專業人員接受了培訓。另外,委員會進行了二項學術研究,以探討專業人員及病人對「康復進程」的態度及接受度。以復康組作為試點,委員會在醫院內推行了一系列有關「康復進程」的活動,以使其逐步推行至醫院內其它服務單位。

總結:「康復進程」的服務模式在中國人精神科服務體系中是可行的。

*「康復進程」乃青山醫院精神科專業人員透過病人及職員的聚焦小組對“Recovery”的中文詮譯。強調「康復進程」是一個個人化的過程,精神病患者在過程中不但學會好好管理自己的精神健康,同時也是一個追求進步的過程,無論疾病所造成的限制是否存在,都可以超越疾病,過著滿足、有希望、又有貢獻的生活。

Recovery-oriented Practice: Essence of Care for Persons with Mental Illness

Abstract
The notion of recovery has spread globally. Recovery is a new paradigm for mental health in many countries and in Hong Kong. Recovery-oriented practice is being promulgated in service provision for persons with mental illness. This model of practice is getting more and more attention by service users and mental health professionals. It denotes a new horizon that mental health professionals need to work with service users in a much more collaborative way than ever. This paper discusses the concept of recovery and recovery-oriented practice, together with the implications for service users and mental health professionals.

以復元為本的服務:為患有精神病的人士提供護理之本質

復元概念近年在世界各地備受關注,復元概念在很多國家和香港是精神健康新思維,以復元為導向之實務被廣傳是為患有精神病的人士所提供之服務。此實施模式備受服務使用者和精神健康醫護人員的關注;它預示一個嶄新的視野:精神健康醫護人員需要與服務使用者前所未有地衷誠一同工作。本文討論復元概念和以復元為導向之實務,與及對服務使用者和精神健康醫護人員的含意。

《復元》運動是精神健康照顧領域上的國際發展趨勢,而新生精神康復會近年來於不同的服務範疇積極推動及實踐以復元為本的工作,包括先導計劃如 「個人復元計劃」、「身心健康行動計劃」及「復元基礎培訓工作坊」等。而在復元系統演進的過程中,職員、康復者及家屬照顧者三方的參與至為重要。

Alliance for Recovery and Care – A Journey of System Transformation
Mental health recovery movement is an emerging international trend in the mental health care field . New Life Psychiatric Rehabilitation Association has been actively engaged in promoting and practicing recovery-oriented practice in its comprehensive service range across the association. A number of pilot programs have been implemented, including Person-centered Care Planning (PCP), Wellness Recovery Action Plan (WRAP), Recovery 101 training workshops etc. Collaborative participation from different stakeholders including staff, people in recovery (PIR) and carers are essential in this transformation process.

Abstract
Peer Support Worker (PSW) project sets a new milestone for psychiatric rehabilitation in Hong Kong. The project, funded by the MINDSET, is collaboratively implemented by four mental health agencies to empower people with mental health problems, and advocate for recovery-oriented mental health care. The first batch of trained PSWs is ready for journeying into unprecedented roles and functions of being a peer support worker. They are faced with an enormous task to demonstrate their uniqueness and irreplaceability in the mental health service system. The challenge of PSW for mental health agencies is a paradigm shift, which requires organizational transformation in embracing the PSWs as valued members of the team.

「思健」朋輩支援計劃-本土經驗及觀察

「思健」朋輩支援計劃開創香港精神康復服務新一頁。四間志願服務機構聯合提倡推行復元為本的社區精神康復服務; 培訓復元人士成為朋輩支援工作員,以過來人經驗,推動其他服務使用者踏上復元之路。首批受訓的朋輩支援工作員已準備開展他們前所未有的工作旅程。他們要面對的挑戰是:如何在其他人面前展示朋輩支援工作員的獨特性及精神康復服務的存在價值。精神康復服務機構的挑戰是:如何以維新變革去迎接朋輩支援工作員作為工作團隊的一份子。

「想說‧相說」-初探香港精神康復服務應用攝影治療的經驗

摘要

攝影治療在西方國家發展由來已久,然而在香港運用攝影作為協助精神病患者的一種治療工具,仍屬嶄新嘗試。香港心理衞生會訓練及就業服務中心(新界)與沙田醫院職業治療部於2012年1至7月舉辦「想說.相說」攝影治療小組,以探討攝影治療在精神康復服務的適用性和治療成效,同時累積在本港應用攝影治療作為治療工具的實務經驗。共30位精神病患者參與攝影治療小組,在三次治療小組完結後,組員被邀請參與半結構式面談及聚焦小組。從參加者的回應顯示,攝影治療小組有助提升組員的自我效能感和改善個人精神狀況。建議在本港持續應用攝影治療以累積實務經驗,並進行廣泛性研究工作,以實證治療成效。

 
Abstract 
An Evaluation of the Therapeutic Photography Program for People with Mental Illness in Hong Kong
Using photography as a therapeutic tool has been employed in western countries for some decades. Yet, in Hong Kong, this is a relatively new intervention for people with mental illness. The Mental Health Association of Hong Kong and Shatin Hospital had pioneered a therapeutic photography program for people with mental illness from January to July 2012. This is the first attempt in Hong Kong to understand the impact of photography as a therapeutic intervention to people with mental illness. A total of 30 psychiatric day patients of Shatin Hospital were recruited. After the program, the participants were invited to join a semi-structured interview and focus group. Participants reported that the program helped them in terms of enhanced awareness on their strength and weakness, and better insight towards their illness. Further applications of therapeutic photography techniques in other settings and large scale studies for its effectiveness in future are recommended.
